Immunization Certificate/MDH Form 896

All campers must provide their immunization records EXCEPT those who attended school (public or private) in Montgomery County, Maryland during 2023-2024.

• US RESIDENTS may submit their Immunization Records in any format provided by their Physician's office.
• NON-US RESIDENTS must submit their Immunization records on MDH form 896. 
• Campers who reside in the US and are EXEMPT FROM IMMUNIZATION must submit their camper's Exemption Certificate on form MDH 896.
  • Medical exemptions must be signed by a physician.
  • Religious exemptions may be signed by a parent.

Allergy Action Plan 

Campers with allergies that require emergency medications, such as Epi-Pens, need to submit the Allergy Action Plan.  Both physician and parent signatures are required. You must provide non-expired, pharmacy labeled epinephrine and/or medication in the original pharmacy container.

Medication Authorization Form

Campers who require daily or emergency medication(s), including over-the-counter medication(s), need to submit the Medication Authorization Form.  The form has space for three medications. Both physician and parent signatures are required. You must provide non-expired medication in the original pharmacy container.

Asthma Action Plan

If your child has asthma and uses an inhaler, the Asthma Action Plan is required with a physician’s signature. You must provide the inhaler in the original pharmacy container with the pharmacy label.
